Before and After Secondary Rhinoplasty

About The Procedure

Dr. Hodgkinson is well experienced in correcting a rhinoplasty, or nose reshaping surgery, that has previously gone wrong this is called secondary rhinoplasty. He sees a large number of patients coming to his clinic after a nose surgery that has been performed elsewhere has led to contour deformities or an unsatisfactory result.

An expert who has been published in major scientific journals discussing the very topic of secondary rhinoplasty, Dr. Hodgkinson often rectifies a common error that results in the over-resect or removal of too much tissue from the nose during an attempt to reshape it.

Dr. Hodgkinson has presented internationally his correction process, which involves the use of bone and autogenous tissues (your own tissue not silicon) to repair over-resection cases in the bridge of the nose.

Dr. Hodgkinson will use the ear and the olecranon or elbow area as a donor site for the surgery which in most cases is performed under a light general anaesthetic. Very occasionally he uses rib cartilage.

A secondary rhinoplasty often involves complicated grafting, and underlying scarring or loss of tissue meaning that the results will be less predictable than primary surgery. Dr. Hodgkinson will always aim for improvement, but all patients must understand that realistic expectations will need to be in mind due to the unique problems secondary rhinoplasties present.

The technique of a revision or secondary rhinoplasty

Rhinoplasty is a very popular cosmetic procedure that does not exist purely to change the aesthetics of someone’s face.

Dr. Hodgkinson uses the grafting technique in instances where too much tissue has been removed by a previous surgeon, requiring cartilage or bone to be grafted from other parts of the body. A similar procedure is used when someone has experienced a severely broken nose. Dr. Hodgkinson will use the ear and the olecranon or elbow area as a donor site for the surgery which in most cases is performed under a light general anaesthetic.

The risk of Injectables into the nose

When looking to correct an over-resect, some surgeons will inject the tip of the nose with fat or fillers. This is an unstable solution that will eventually see a loss of shape and form, requiring it to be repeated continually. This technique can also lead to further destruction of the tissues making the problem far worse in the long run and in severe cases, even causing blindness due to fat embolism.

As a result, Dr. Hodgkinson does not advocate for this technique, instead opting to use tissue from behind the ear (posterior auricular fascia) to create a pad that can be placed under the skin on the tip of the nose. This creates a soft and natural contour which is also suitable for use in the saddle of the nose to correct a contour deformity in this region as well.

By using the patient’s own tissues, the risk of an adverse tissue reaction is removed. There will be no visible scarring at the donor site behind the ear as the incision can be placed in the crease.

Dr. Hodgkinson has previously demonstrated that this technique is clinically sound, and leads to excellent long term results in a number of cases. His results have been published in the peer reviewed Plastic Surgery literature.

Dr Hodgkinson’s experienced team put a significant focus on the pre and post-care of a patient for any procedure. How do I know if I need a secondary rhinoplasty?

The results of a rhinoplasty procedure that has been performed correctly should last a lifetime and should only be performed once. The correct aesthetic of a nose job should be appropriate and individualised for each patient.  While rhinoplasty is a procedure that aims to reshape the nose to provide a pleasing appearance. There are some cases where patients are dissatisfied or experience issues with a nose job that create the need for a revision. This is often obvious when the nose is not in harmony with other facial features, especially in profile view, this may result no matter how much care is taken in the operation, as healing differs from patient to patient.

Well-performed rhinoplasty procedures should also be ethnically appropriate and individualised to the other facial features, appearing natural without any visible evidence of surgery. The “nose job look” stereotypically features a nose reduced in size and too small for the face. Some surgeons think a “standard” nose looks good on all patients but Dr Hodgkinson believes there is one nose just right for each patient and it differs from patient to patient.

Why you must be selective of the surgeon performing your revision procedure

Secondary rhinoplasty is for expert nose surgeons only.

Revision rhinoplasty is much more difficult, time-consuming and less predictable, with a risk of the ‘cookie-cutter’ look if left to the hands of an inexperienced surgeon who can create a desirable look for both men and women.

Dr. Hodgkinson is not only experienced in the revision procedure, but also uses a method that involves the patient’s own tissue. This helps avoid long-term complications such as extrusion and infection of foreign material if left in the nose. Healing in the nose is very unpredictable, and slight asymmetries are very common after about three to six months. You should be aware that a minor touchup procedure may be required after your first rhinoplasty. Secondary rhinoplasty results are even less predictable.

Dr. Hodgkinson will always aim for improvement, but all patients must understand that realistic expectations will need to be in mind due to the unique problems secondary rhinoplasties present.

Frequently Asked Questions

  • Will people be able to tell that I’ve had a nose job?

This is often one of the largest fears faced by patients when it comes to having a nose augmentation. Dr. Hodgkinson’s specialty across all procedures is a sensitivity to a range of elements. This includes the look of the person’s face, as well as gender, ethnic background, and family resemblance. A good result is a nose nobody notices as it blends and harmonises well with your other features, especially your profile.
