A Critical Part of Pan-Facial Rejuvenation

The neck is an area that is often overlooked as part of facial rejuvenation and yet the ageing of the neck is one of the most frequently mentioned areas of concern by patients from their 40’s and upwards when seeking advice about rejuvenation procedures.

One area that has never been properly addressed is the lower part of the neck and how the whole neck needs to be assessed in conjunction with the face when undertaking any form of facial rejuvenation, especially facelifting, browlifting and even eyelid surgery.

The most natural overall result

The concept of “Pan-Facial Rejuvenation” is that all the aesthetic areas of the face are addressed simultaneously to create the most natural overall result possible.

I was invited to present my concept of Total Neck Rejuvenation at the recent biannual meeting of ISAPS (International Society of Aesthetic Plastic Surgery) in Geneva, Switzerland. This represents an improvement on traditional techniques, which have mainly concentrated only on the upper third of the neck, defining the jaw.

Although it is very important for a woman to have a nice jawline, women also want a tall neck, a neck without horizontal wrinkles and a neck for which the definition goes all the way from the collar bones up to the ears.

Symbol of Beauty and Youth

Since time immemorial, a tall, taut neck has been a symbol of beauty and youth. The statue of Nefertiti stands as an icon of beauty, even though Nefertiti lived 3000 years ago.

Throughout history, artists have portrayed beautiful women with a long, tall neck and women accentuate this by wearing clothing that shows their décolletage and enhancing their neck with jewelry. In some cultures, the neck is lengthened also by artificial means.

The famous French artist, Jean-Auguste Dominique Ingres said and I quote:”a woman’s neck can never be too long”.

For many years, the techniques of surgery, certainly improved the definition of the jawline and the upper part of the neck, but I found when my patients were complaining about their neck, they grabbed hold of the tissue in the bottom half and wiggled it – what we would refer to as a “turkey neck”.

The Fogli technique which I have been working with for 6 years after much discussion with Dr Alain Fogli, an innovative surgeon from the south of France, has offered my patients a unique opportunity to have this tall, taut neck.

All the lesser procedures like threads, short incision scar facelifts do make improvements but do very little to the neck and patient dissatisfaction is usually quite high after the minimal incisions or “lunchtime lifts” which can result in a tight, “operated-on” look.

There will always be a place for proper facial rejuvenation surgery and the value of a procedure or procedures which give not only dramatic but natural results that can last for over 10 years.

ISAPS Meeting in Geneva, an invaluable experience The International Society of Aesthetic Plastic Surgery (ISAPS) holds a scientific meeting in different continents every two years. This year it was hosted by ISAPS’ now past President Jan Poell in Geneva in his native Switzerland.

An international surgical faculty representing over 40 countries and 200 surgeons presented lectures, video sessions, case presentations, symposia and panel discussions to more than 1400 international delegates. Dr Bryan Mendelson and I were the Australian faculty members and both of us lectured in the same session on facelifting.

To share our experiences with such a prestigious faculty from all over the world is a unique experience and provides a great boost for any surgeon who makes the effort to attend or prepare papers for these international meetings. To be invited to be on the faculty was indeed an honour.

The next ISAPS biannual meeting is in Brazil in 2014 and this will be hosted by Dr Carlos Uebel, ISAPS’ newly elected President whom you see in the picture with me and some other international surgeons who attended this year’s ISAPS meeting.

Dr Hodgkinson answers:

A: Yes, breast implant surgery is safe, but women who are interested in this procedure do need to be informed about their options. With the recent PIP implant scare, women are concerned about implant type. Whenever possible I prefer the sensible “headache free” breast augmentation – Saline Implants (as opposed to gel or the Brazilians).

Patients need to be involved in the choice of implant. Many patients want to speak to someone who has had implants. Some of my staff have had saline implants inserted by me years ago and continue to be happy with their results; they would be happy to speak to you.

– Not a “Nose Job”

I continue to enjoy rhinoplasty surgery after more than 30 years of performing this most technical and demanding operation which provides great satisfaction for patient and surgeon alike.

My contemporary rhinoplasty is constantly evolving not only from my own experience but from attending as well as lecturing at rhinoplasty meetings.

Innovative ideas and techniques

I am proud to have recently been made a member of The Rhinoplasty Society, an organization dedicated to the open exchange of innovative ideas and techniques concerning rhinoplastic surgery which consists of surgeons throughout the world who specialise in rhinoplasty. I am one of only two members in Australia.

All rhinoplasties need careful pre-operative planning which includes computer imaging to understand what can be achieved and what the patients’ aesthetic desires are. There is no standard or one good looking nose that suits every face.

What I aim to do is customize the rhinoplasty to be in harmony with all the particular facial structures; (eg brow, chin, lips and neck and the overall personality of the patient). Given this, multiple consultations can be necessary to achieve an understanding between the patient and the surgeon.

Breathing function also tested

Prior to surgery in addition to imaging, I test the breathing function of the nose and sometimes X-rays are necessary to discover the cause of nasal obstruction. In most cases any difficulties with breathing can be improved at the same time as the rhinoplasty surgery.

The procedure itself is carried out on an outpatient surgical basis at the Double Bay Day Surgery. A popular time for this surgery, especially for students or people with leave from work, is over the Christmas and January summer holidays.

Rhinoplasty, however, is performed on all age groups for both men and women. In the over 40’s, rhinoplasty is becoming more commonly combined with facelifting as a part of pan-facial rejuvenation, as an attractive nose which fits the face also contributes to making a face look younger and balanced.

Liposculpture has not only remained popular for men and women alike since its inception over thirty years ago when I began performing this procedure, but has actually become more commonly used as people in developed nations struggle more and more with their weight and shape.

As summer approaches, we realise that we are not going to have the beach bodies we want with the extra a kilo or two that we have gained during winter.

As we age, the fat is more difficult to budge and these fatty deposits tend to be in areas which are hard to tone or in which the person is genetically predisposed to carry fat.

In men, we are talking usually about the abdomen and the hip area as well as “man boobs”. In women, we are generally discussing the inner thighs, the outer thighs, the hip areas and sometimes the tummy and arms. These areas can all be addressed in one stage.

After careful planning and design taking into account the patient’s entire body shape, the procedure is carried out under light general anaesthetic. Patients wear compressive garments for 4-6 weeks post-operatively to optimize their results.

Of course as in all surgery, the most important factors in the result are primarily the skill of the surgeon and the compliance of the patient with pre and post-operative instructions.
