    Rhinoplasty in males is now quite common representing 25% of the approximately 500 cases I perform each year.  Often men have a history of nasal trauma from contact sports or accidents in their childhood or young adult life which in turn affects their breathing.

    On consultation we always test the breathing function and evaluate the inside structures as well as using computer imaging to determine what the best shape of the nose should be to complement the overall facial features. Sometimes to further evaluate the breathing we may need a CAT or MRI scan to aid in the planning.

    Once the patient and I agree on the possible shape of the nose, we can proceed with the surgery which is carried out as an out-patient procedure at our on site fully licensed and accredited facility, The Double Bay Day Surgery.  The entire surgery including the correction of the breathing usually takes about one and a half hours.  A splint is worn on the nose for one week and most patients go back to work or their studies after seven to ten days.  Any discomfort is easily managed with mild analgesia.

    If a patient has had an unacceptable result from another surgeon and requires secondary surgery, the results are less predictable.

    As the warmer weather approaches, we are all a little concerned about the few kilos which have accumulated over the winter. Diet and exercise help, but for most of the patients seeking liposuction, there are areas that they just can not budge.  For men it is usually the tummy (beer belly), love handles and man boobs.  For women it is the hips and thighs which give the greatest concerns.

    I have performing liposuction/liposculpture for 30 years and find this procedure the most gratifying for these patients who are challenged by their inability to change the patterns of their fat distribution.  We can dramatically improve their shape but we have to recognise that the results of liposuction will not last if the patients regain weight in the future.  Pregnancy, IVF and menopause all alter the hormones and metabolism and will negatively impact on any liposuction result.

    My advice is that is now is the time to be informed by having a consultation to see if you might be a candidate for liposculpture so that you will be ready for summer as the optimal result is not seen for 6 or more weeks.

    Surprisingly the procedure is not painful and the recovery period off work is generally only a few days to a week maximum.

    Breast augmentation remains very popular at our facility as it is worldwide.  New implant products have come and gone but the saline implant and the newer gel implants are my preference and satisfy 90% of patients.  There are a wide variety of shapes which need to be fitted to the patient’s chest shape and breast characteristics.  For Asian patients I use the arm pit incision and for Caucasians a small inframamary incision under the breast allows for insertion of the implant. Generally the upper border of the implant is placed just under the pectoralis muscle to avoid implant show often referred to as the subpectoral approach.

    After sizing is carried out, respecting the patient’s own choices then advising her on the most appropriate way to achieve a natural result.

    The operative procedure is carried out under a light general anaesthesia and no drains are used.  The recovery usually without incidence usually allows the patient to return to work in about one week.  There are no stitches to remove and the patient is provided with a complementary support brassiere perfectly fitted to her new breasts.

    Until new implants which come onto the market are proven long term to give a safe and superior result, I use the safest implants that are TGA and FDA approved in the USA.  50% of my patients choose saline and the other 50% are silicone implants.

    Latest trends in cosmetic surgery at ISAPS Brazil

    The 22nd biannual meeting of the International Society of Aesthetic Plastic Surgery met in September in Rio de Janeiro in Brazil for four days of intense education on the latest trends in cosmetic plastic surgery. I was fortunate to be chosen to deliver two papers on the face and neck lift, being one of only two Australians on the prestigious international faculty representing 76 countries.

    My paper was enthusiastically received and it is gratifying to know that our dedication to the best surgical results are recognised by international peers.  Interesting, the emphasis on facelifts is different between the Europe the USA and Australia.  The Americans want a more dramatic, even exaggerated result, the Europeans don’t focus much on the neck whereas my Australian patients want a natural looking result with very good improvement on their neck.

    In 1993 Dr Hodgkinson, who had already been active in charity surgery for many years around the world in Guatemala, Mexico, India, Nepal and the Philippines, was approached about the great need amongst the poor children with cleft lip and palate in Cebu, the second largest city in the Philippines.  His response was to found Operation Restore Hope which began carrying out surgical missions in 1994.  Since then, Operation Restore Hope has carried out nearly 2,500 surgeries and provided ancillary health care services especially dental to countless others in the 45 missions it has fielded in both Cebu and Luzon.
